What is Reverse Tubal Ligation? Reverse tubal ligation - or more properly, tubal ligation reversal - is a procedure to get tubes untied for women who desire a pregnancy after tubal ligation. There are actually several procedures that can be used to untie tubes after the tubes have been tied.

Techniques of Reverse Tubal Ligation

Microsurgical tubal anastomosis is the most common technique to untie tubes.

There are 3 main techniques that can be used for reversing tubal ligation.
  1. Tubal anastomosis
  2. Tubal implantation
  3. Salpingostomy (Fimbriectomy Reversal)

Tubal anastomosis is the best procedure to get tubes untied.Tubal Anastomosis

The most common method for untying tubes is the reverse tubal ligation procedure of tubal anastomosis. Anastomosis refers to joining two body parts, and tubotubal anastomosis is joining two tubal segments together. Most techniques that tie tubes result in two separate tubal segments, so the simplest way to get tubes untied in these cases is with the anastomosis procedure.

What is the Cost of Untying Tubes?

Reverse tubal ligation is usually described as extremely expensive, ranging from $10,000 to $30,000. The tubal anastomosis procedure to untie tubes costs $5900 at Chapel Hill Tubal Reversal Center. Because it is done as outpatient surgery and performed four times a day, the cost savings is passed on to the women who want kids after tubal ligation.

Reverse tubal ligation (tubal ligation reversal) is a procedure to get tubes untied for women who desire a pregnancy after tubal ligation. There are actually several procedures that can be used to untie tubes.

The three most common tubal ligation reversal techniques are tubal anastomosis, tubal implantation, and salpingostomy. These procedures are used to reverse several different types of tubal ligations, including ligation/resection, coagulation, fimbriectomy and tubal clips or rings. Why Untie Tubes?

New Pregnancies Announced This Week at Chapel Hill Tubal Reversal Center

The Weekly Pregnancy Report for Chapel Hill Tubal Reversal Center is published on the Tubal Reversal Message Board. During the week of April 19 – April 25, 2008, 20 new pregnancies were reported by Dr. Berger’s patients after their tubal reversal procedures. The women’s ages ranged from 33 to 40, and the fallopian tube lengths after their tubes were untied ranged from 3.0 cm to 7.5 cm. The tubal ligation methods among these women included Hulka clips, Falope rings, bipolar coagulation, and ligation/resection. At present, more than 1000 babies have been born following tubal ligation reversal by Dr. Berger.

Remarks: I’m not entirely sure how this happened!!! LOL - I must have ovulated right after my period stopped??? I don’t really ‘feel’ pregnant yet, the only reason I tested was because I thought I started bleeding/spotting again 2 days after my cycle stopped. Then it stopped after 2 days and started again 2 days later. I’ve never ever had spotting or any kind of break thru bleeding-except when I was pregnant. Also, I was having pain on my right side that was strong enough to wake me from sleep Friday night. We did the hpt and that line came of fast. We aren’t talking a faint line, like I would have expected on CD16!!!(if any line at all) My Dr. sent us to the Hospital since I was having pain on Rt. side. The pain went away and my hcg level was 549. Although it was too early to see anything, they did an ultrasound anyway. Nothing. I test today for 2nd level. I still have some spotting, not like I did on Saturday. Just playing the waiting game and praying for our miracle. Funny how life changes in a moment! Thanks!!

Remarks: I have had 2 miscarriages and one ectopic (lost my right tube). We weren’t trying because my HSG showed scar tissue even though I did see with my own eyes the dye go all the way through. We weren’t TTC since I was told I didn’t ovulate without Clomid (I have never test positive for ovulation with test sticks or a fertility monitor without being one Clomid). We are in total shock and of coarse overjoyed and scared at the same time. My 1st HCG number at CD 31 was 21 at CD 33 39 (that was 45 hours apart). I will keep you updated.

Remarks: I used ovulation test sticks (7 stick packs), monitored daily temperature and monitored my cycle. In March (period began 3/1) the peak ovulation day was never indicated. In April I had what appeared to be a very brief (2 1/2 days) period beginning 4/1. Noticed swollen and tender breast on 4/10. Began my usual ovulation stick test on 4/11. On the morning of 4/13 I took both an ovulation test and a pregnancy test and the pregnancy test came up positive. Noticed some spotting that day. Had a quantitative analysis done at ob/gyn on 4/14 which confirmed the pregnancy. Monday 4/14 Progesterone 3.87 and HCG 144.1. Due to the low numbers I was put on Progesterone suppositories and pelvic rest. Wednesday 4/16 Progesterone 22.25 and HCG 176.8. Today, Friday 4/18 HCG 299.00. I continue daily spotting and remain on Progesterone suppositories. I am scheduled for another quantitative analysis on Monday 4/21.

Dr. Caryn M. Hertz - Director of Anesthesia

Dr. Caryn Hertz is Director of Anesthesia at Chapel Hill Tubal Reversal Center.

Caryn M. Hertz, MD is a Board Certified Anesthesiologist and has worked in the field for over 20 years. She has devoted her professional career to the subspecialty of ambulatory (outpatient) anesthesia. She has been working with Dr. Gary Berger at Chapel Hill Tubal Reversal Center since 1995.

Originally from Queens, New York, Dr. Hertz attended college at The Cooper Union for the Advancement of Science and Art, graduate school at The University of Pennsylvania, and medical school at the University of Rochester. She completed her residency in anesthesiology at Beth Israel Hospital in Boston (a Harvard affiliate) and at the University of North Carolina at Chapel Hill. After finishing her residency, she was employed at Duke University Medical Center for 5 years, specializing in preoperative assessment and ambulatory surgery. Dr Hertz has worked at Chapel Hill Surgical Center ever since.

Dr. Hertz says, “I am dedicated to making each patient’s surgery comfortable and safe. Working regularly with Dr. Berger has enabled me to truly optimize and continually improve the Tubal Ligation Reversal experience for our patients.”

Dr. Hertz and her family have lived in Chapel Hill since 1989. Having relocated from the Northeast, they very much enjoyed the mild weather, beautiful environment, and remarkable community here in Chapel Hill.

Chapel Hill Tubal Reversal Center Welcomes Dr. Split


Dr. James Split, a board certified anesthesiologist, is the newest member of the Chapel Hill Tubal Reversal Center staff. Although only 4 tubal reversal surgeries are performed each day at Chapel Hill Tubal Reversal Center, there are 2 anesthesiologists and a nurse anesthetist on staff. The primary responsibility of the anesthesia department is to ensure patients' safety during surgery and comfort during recovery. Many of Dr. Berger's patients who are having their tubes untied will be speaking with Dr. Split as a part of their preoperative evaluation and will meet him on the day prior to or on the day of their tubal ligation reversals.

It is a happy occasion to introduce another new member of the staff of Chapel Hill Surgical Center. Dr. James Split is a Board Certified Anesthesiologist. He has worked at Chapel Hill Surgical Center since December 2007. He previously lived and worked in Greensboro, North Carolina for the last 18 months. Before moving to North Carolina, Dr. Split practiced medicine in Michigan for 25 years. He obtained his medical degree at the University of Michigan in 1979 and practiced Emergency Medicine for the first 11 years of his career. He both practiced and taught Emergency medicine in an Emergency Medicine Residency and held a clinical faculty position at Michigan State University. In 1990, he returned to the University of Michigan to train in anesthesiology. He completed his anesthesiology residency in 1993 and has been in the full time practice of anesthesiology since then. Dr. Split is a member of the American Society of Anesthesiologists and The Society for Ambulatory Anesthesia.

Dr. Split will be speaking with many of you as part of your pre-operative evaluation. Dr. Berger’s Comment

The highest priority for patients who come to me for tubal reversal surgery to untie tubes is patient safety during and after surgery. This is ensured by the involvement of medical professionals who are the most highly trained and qualified specialists. Our two anesthesiologists are certified by the American Board of Anesthesiology. The anesthesiologist is the physician responsible for the monitoring and care of patients while they are asleep during their tubal reversal procedure and for their safe and comfortable recovery in the post anesthesia care unit (PACU).     Patients are often very apprehensive about their anesthesia for the tubal reversal surgery. Dr. Split is extremely friendly and I think he does a great job of putting patients fears aside and helping them to feel comfortable about their upcoming procedure.

Tubal Reversal Surgery at A Discounted Rate

Dr. Charles Monteith will be joining the staff of Chapel Hill Tubal Reversal Center as a Tubal Reversal Specialist in July of 2008. By then, he will have trained with Dr. Berger in tubal reversal surgery for six months. The initial fee for surgery with Dr. Monteith will be a discounted rate of $4900. This rate will be available from July 21st to September 1st. This is an all-inclusive fee and includes all aspects of care. Many patients wanting to have children after tubal ligation are taking advantage of the reduced rate with Dr. Monteith.

I was just wondering if Dr B ever run any monthly specials. I know the tr is less than most, but just was curious if he would do something special for his potential tr patients. That would be nice if he did just thought I would ask. Hasty

I don't know how often, or if he even has done it before, but when I called late Nov '07, he had a special going, and I got the last spot available for the reduced amount, Dec 31 '07...by paying in full within 24 hours. Dawn M.

Hasty, I think if there is a special being ran you would find out when you call, I don't believe they tend to advertise specials. However, I know the other doctor was offering TR at a reduced price but I am not sure if he still is! GL!

The Time Between Tubal Ligation and Tubal Reversal Does Not Affect The Pregnancy Success Rate After Tubal Reversal Surgery

The time interval from tubal ligation to tubal reversal has not been shown to affect pregnancy success rate after tubal reversal surgery. Tubal reversal surgery can be done on women who have had their tubes tied twenty years ago or less than one year. There are many reasons that cause a woman to change her mind and desire more children after a previous sterilization. The desire for more children can happen immediately after tubal ligation or it can happen months or even years later.

Ok I had a HSG done today. I don't have the "official reports" yet. The doctor said I have about 2 inches on each side.
My type of TL was a fimbriectomy. So do you think I can have a TR?

Anxious...nope no time limit! Send your OR from your TL to Dr B for his review..... the time between my TL and TR was 11 years! There's ladies here with a greater gap between the two.

Tubal Reversal Sucess Rates

People often wonder "Even if I do get the tubal reversal procedure done, what are the chances I'll conceive?"

Dr. Berger of Chapel Hill Tubal Reversal center is the only doctor who tracks his specific success rate – others doctors will just quote the industry average. Dr. Berger has taken the time and the expense to meticulously keep track of patients pregnancy and birth rates and make sure the procedure is as pain-free as possible.

One way to determine what your success rate may be is with the operative report from your tubal ligation. This can be obtained from the doctor who performed the surgery or from the medical records department of the hospital where your procedure was performed.
